Registration Form for Teen Summer Reading Participation

1. Any teen (rising 6th grader to 12th grader) may sign-up to participate.

2. Participants should read 7 young adult or adult books between June 8th and August 4th and complete a Reading Record Sheet.

3. Each person who registers will be entered into a drawing for various prizes during the summer. Names will be drawn on June 16, June 30 & July 28.

4. All participants who turn in a completed Book Review Form to the 2nd Floor Desk by August 3 will be entered into the Grand Prize drawing. You may turn in more than one sheet. Those who attend program events will also be entered into the Grand Prize Drawing. The drawing will be held in August 4th.
